Has anybody a sample site assessment plan?

Has anyone come across a good definition of "regularly used exterior entrance"?  Such as entrances used at least once a day or once a week?

Isn't it correct that LEED changed their interpretation of this credit so that if No off-street parking is provided, No credit is given?
