Where is form Plf3-1?

I'm looking for the form required to comply with "Complex Occupancy" under form PLF3. I have a Rec Center with low full staff occupancy (5) but VERY HIGHT Peak transient occupants (642).

If a FloorScore certified product, specifically a pre-finished engineered wood floor, is applied to a wall or ceiling application does the product contribute to the EQc2 credit in LEED v.4?

Interchangeability of LE/FE vehicles and HOV parking spaces

We are pursuing both SS4.3 and SS4.4. As long as we provide the total parking spaces required for both 4.3 AND 4.4, can those spaces be used interchangeably for either LE/FE and HOV?

Why am I penalized and unable to achieve these credits simply because we decided an air cooled chiller was more energy efficient, and my clinic doesn't serve food?
