What is the way to carry out the surveys in a Hotel, where there are fixed staff and guests?

Is there a standard LEED users' survey form for the thermal comfort verification or is this something that needs to be generated by the Owner for each project?

I have a hotel + residential tower project with approx 50-50 break up. Can this project achieve Thermal Verification credit?
