Can anyone clarify if the maximum allowed percentage of total lumens emitted above horizontal (by lighting zone) is applied to each individual luminaire or the sum of all lumens from all luminaires inside the project boundary? Thank you.

It is total lumens for the sum of all lights within the project boundary. One or more fixtures can exceed the % as long as the total for the site doesn't.
From a practical point of view, these values are so low that having one light well above the % can make the whole site difficult to show compliance. This is why most projects have all lights below the % threshold.

Does that mean a flag other than the national flag (such as state or city flag) could be uplit so long as the total for the site doesn't exceed the max?

If you're doing the calculation method, yes. Do anything you want with lights as long and you can show the site as a whole is compliant.
The exemption only covers the national flag. Lighting for the other flags will be counted. Having a limit of only 1.5% or 3% does get difficult. You'll need a lot of full cutoff lights to offset the flag lights. It's possible, but it is hard for small to medium size projects because there are so few total lights.
