If the audit was conducted in 2014 and we are submitting the LEED application in 2019, does the performance period start date need to be in 2014? How long is the performance period allowed to be? Do all low cost measures need to be completed within the performance period?

The audit must occur during the performance period. The performance period can be up to 24 months long. So if submitting in 2019 you could have performed the audit back in 2017. Any audit performed before then would need to be updated.
No-cost / low-cost measures must be implemented during the performance period.
My advice, update the Level I energy audit and implement the no-cost / low-cost measures then submit the LEED application.
The owner determines what is considered to be 'low-cost' and 'capital improvements'.
