Sorry, I haven't been in this situation. My basic advice for wacky forms is always 1) check that it's not a BETA form, and 2) contact GBCI via the feedback link.

Thanks, Tristan. The form isn't tagged as BETA. I did contact GBCI but haven't gotten a direct response. Hopefully soon.
