General Contractor has provided LEED submittal material form but the backup information ranges from a job specific letter from the manufacturer, to information from a manufacturer web site. Is this form typically enough for LEED Reviewers to accept and approve, or do they require actual invoices to indicate product costs?

The documentation required here is cut sheets or similar documentation demonstrating recycled content. Invoices are not required, to my knowledge.
