Yes, I just located it. It's under the "Legal" link on LEED Online, or here is a direct link to download it.

I'm fairly certain it was not there before, since we contacted GBCI and they acknowledged that it was not posted. But I didn't know that they had fixed it. Thanks!

I have another question about this form. How do you interpret the following sentence in the form:
“If someone other than the owner of the project is, or will be, submitting materials, executing documents and agreeing to various obligations, GBCI requires explicit confirmation from the owner that these actions are being taken on such Owner’s behalf by an authorized party.”
My initial understanding of this form is that it would allow a party like the design professional to sign LEED agreements on behalf of the Owner, and possibly to complete Owner-related LEED documentation and signatories throughout LEED Online (with Owner review/approval of course). However that sentence could be interpreted that ANYONE other than the Owner doing any part of the LEED application documentation (Engineers, GC, etc.) needs to have this form executed between themselves and the Owner. I hope that is not the intention!

Carrie, I'm sorry I haven't gotten back to you about this sooner. The sentence you're quoting is in the clause of the document labeled "Document Purpose." So I think the sentence describes exactly what the document is supposed to be doing.I don't think it goes as far as you fear relative to anyone doing anything with the LEED documentation process. What are your thoughts?
