Wondering if a CS industrial project (cold, dark shell) with unknown tenant must include a vestibule in the CS design, or if it can be assumed this will be provided as part of the tenant fit-up along with the other tenant fit-up related ASHRAE 90.1 mandatory requirements?

Depends on the design. If there is clearly a space intended to be the "office" with a clear entrance then a vestibule makes sense and since it is a part of the envelop being constructed as a part of the shell it would make sense to include it. Some of the other "entrances" to a large building may not be clearly "entrances" without an interior layout so you could probably lump them in with the fitout work.
