The Staff would more likely participate in a survey that is only two pages like the LEEDuser Sample of an accepted survey as opposed to the Template offered in the Toolkit. Is this really acceptable? It seems like the LEEDuser Template might be overkill, however, we need the credit and do not want to use something that will be rejected. Basically, theLEEDuser Sample has a list with a scale from -3 to +3 that looks like this:
Satisfied Mostly Satisfied Somewhat Satisfied Neutral Somewhat Dissatisfied Mostly Dissatisfied Very Dissatisfied

Section 1 – Background information.
Section 2 – Current thermal conditions in your space
Section 3 – Thermal conditions in your space during the winter months
Section 4 – Thermal conditions in your space during the summer months.
Section 5 – Acoustical conditions in your space.
Section 6 – Lighting conditions in your space.
Section 7 – Air quality in your space.
Section 8 – Cleanliness / maintenance in your space and the general building.
Section 9 – Overall satisfaction with building performance.
The last section is a box for additional comments.

- The scale (-3 to 3) is the recommended format and will not likely be accepted using any other scale.
- In my experience, the necessary questions are limited to light, IAQ, cleanliness, acoustic, general, workspace and thermal comfort satisfaction. Anything beyond that is a bonus and should ONLY be asked if it will help the building operator make people more comfortable. Background info and the fractured thermal comfort questions are not necessary.
My survey is only 1 page, and may be compressed to a half a page and leaves a very open ended section for those comments addressing negative feedback - as it should. After many reviews of using tried and tested methodology - it has never once been subjected to denial, but I will leave this for others to comment about as well. I hope this is helpful, it was a great question - glad to be part of the conversation!
