Substantial differences between the CGP from 2012 as required in LEED V4?

Can anyone please tell me how different should be documenting this credit comparing with LEED 2009? I had a typical document for submission developed by myself, but I am not sure on how to update it.

Veronica, the requirements are virtually unchanged. I haven't checked the documentation, but I would expect the same there. You can check the sample LEED Online form here.
