I would like to know if the signature of the form must be of the owner of the building, or if it could be of the constructor.

Contrator or Architect, as the owner is not (usually) involved in the specification or application of these products.

I'm aware that the previous version of the form (v3) clearly indicated that the signatory should be made by the contractor, however, in the newest version (v4) this information isn't present in the form. It is only stated there that "All adhesives and sealants used on the inside of the weatherproofing system and applied on-site have been included in the tables above" but it doesn't indicate who has to sign this statement. Can you please confirm this?

My understanding is that many of the signatory requirements have gone away. In some cases they were causing undue hassle and problems for project teams. I can't speak to this particular credit, though.
