I understand that there were not any requirements regarding TAB in LEED 2009 -- is this still true for v4? Just want to confirm that there are no issues with having the balancer contracted under our construction manager.

LEED v4 does not have a contract language with respect to the balancing contractor and who they can/cannot be contracted through.

LEED does not go into that level of detail in regards to balancing.....but remembering back in my HVAC design days a long time ago, I never recall a project that 'sampled' the test and balancing. Every system level (e.g. air handling units) every zone level (e.g. variable air volume boxes) and every diffuser/fan/pump were included in the balancing.
Commissioning tends to sample 10% to 20% of all zone level equipment while commissioning all system level equipment. Commissioning Authority will probably spot check airflows at some of the diffusers/grilles.
