Template for a Waste Management Plan

I am looking to provide guidance on a school demolition that needs to conform to a Waste Management Plan, for LEED credits. I understand that the LEEDUser website has templates for a Waste Management Plan. I wanted to view one, or part of one, before I proceeded to purchase one.
