The CxA shall prepare, review with the contractor/supplier, and verify the funtional performance test forms. The CxA shall prepare forms for all systems to be commissioned. This should include, at a minimum, the systems required by LEED, but may also include other items as directed by the owner in the contract documents and OPR.
