Has anyone else noticed that the sample template downloads isn't working. The page is blank? Neither the link from here or directly from LEED Online.

Kathleen, I'm not having trouble with either link.You do have to open the sample forms in Adobe Reader, so if your web browser isn't set up to do that, you might not be seeing the forms.
