Required Signatory IEQp2-1 form?

On the form for this credit, there is an option to upload IEQp2-RS1. Quoting form: "Provide a document with the statement copied directly from Required Signatory IEQp2-1, signed and dated by the owner on letterhead." Where do we find this form?

Romina,I have not heard of a specific form for this. You can just take a screenshot of the portion of the form the required signatory needs to sign, including the statement they are agreeing to. Put that image on a word document and have them sign it. You can then upload that as a PDF. Hope that helps!
