Required signatories of whom?

In the newest version (v4) of the LEED forms and specifically on the form for this credit there are several required signatories, however, there isn't any information regarding of who is supposed to sign them. I remember that in the previous version (v3) of the LEED forms this information was provided next to the field where the team member is suppose to enter his initials. Are you having the same problem? Thank you.

I think the latest version of this form is V5. I believe that anyone can check the boxes on the newer versions.
