Do the electric vehicle parking spaces need to be located within a certain distance of the building entrance? If yes, what is maximum allowable distance and is this distance a radial distance or actual walking distance from space to building entry?

The v4.1 credit does not require a distance to building entrance. The EV-charging/parking spaces can be located anywhere. as a side note, we use the July 2019 edition of the v4.1 rating system ...since this only requires a 2% threshold for EV charging stations rather than 5%.
