The form requires you upload Plf3-1 but i don't know where to get that table

Same problem for me. If I figure this out, I'll post the solution here.

The Template IS PLF3 and PLF3 -1 , -2 & -3 are parts of it. Specifically PLF3-1 is the Spaces area and usage data, which you enter. There is no other form needing upload. Or am I not understanding the question.
