The perimeter light levels are driven by the surrounding light sources such as street lights and adjoining building lights. Do I need to make note of these factors in the credit form?

If you are following option 2, you measure the perimeter twice. Once with the project's exterior lights on, and a second time without. Both measurements will include the sourrounding light sources as you noted. The purpose of this option #2 in the credit is to show the surrounding light sources makes up the vast majority of the light on the perimeter, and that this LEED project makes up a small portion.
