With respect to calculating the "Square footage of all parking areas" . . . is that square footage calculation to include the paved route to access to spaces such as in a parking lot situation? Is the square footage to include the roadway as well if parallel parking spaces provided adjacent to a roadway?
OR - is this square footage to simply be calculated as the actual square footage of the parking spaces themselves (for example 9'-0" x 18'-0" for each parking space) - not counting the access?

We typically include paved roads in the "parking areas". That way the areas of the building footprint, pedestrian hardscape (SSc5.2), parking, and vegetated open space will add up to the total LEED site area. It used to be that the information on PIf2 and SSc5.2 are linked; not sure if that is still the case.
