I am preparing to conduct the occupant comfort survey in a hospital.
I am not sure if the below form is valid:

I noticed the template in documentation toolkit is different & more detailed.
The recommendation is to use a "brief survey".
Due to the number, diversity of functions & workplaces of the hospital's occupants,
the above form seems to be more practical.
It is our first survey, & I am not sure if it would gather enough information for appropriate action plans & meeting credit requirements.
Any help in that regard would be appreciated.

This looks adequate to me- I have always found a simple, direct survey to be best. I would suggest that you add a description of what each value on your -3 to +3 scale represents, though, since this scale is likely new to those taking the survey.

Hello everyone,
I am trying to implement a comfort survey for worldwide office occupants. I am planning to use the detailed template, but it may be too long to answer, and as a consequence not as relevant as expected.
Do you usually use the detailed version? What are your feedback (very useful, very detailed, too long to answer,...)? Have you implemented a shorter version (like the one mentioned by Aurélie Kazour)?
