Danny, the form you're referring to has been an integral part of LEED-2009 MR documentation, but there is no equivalent for v2.2.
We do offer something like this through LEEDuser -- check out the materials tracking spreadsheet in our MRc4 Doc Toolkit. You will still have to fill out the LEED Online form at the end of the day, but this is a useful tracking spreadsheet for all the MR credits.
