I'm wondering if anyone knows what specifically is needed in order to get the Heat Island credit? The project has a 5 story, above ground parking garage without a roof on the top level. The main office building is covered with solar panels. Which option would be best to pursue for this measure? What do I need?

Depends on your site. For the parking garage, sounds like the top layer is regular asphalt and would not help your compliance. This would need to be balanced with the rest of your site hardscape/vegetation and the roof reflectivity wherever there are not any solar panels or other excluded areas, such as other mechanical equipment, openings, etc. You may not have much roof area remaining that needs to be compliance.
An easy way to determine outcome is to use the credit form (either on your LEED website or you can download a free one from the Credit Library), enter the info it lists, and see where you end up.

Have you seen the form? The credit form has no information to fill out. You select which option you want, select both for exemplary credits, sign and date. This is why I reached out to the LEEDUser Forum. I was hoping someone could help?

Yes, I use the forms regularly on numerous projects, and often work with our arch team members specifically on this credit.
I downloaded the sample form from the Credit Library to make sure it's functioning the same way it does on an active LEED Online project website, and it all looks the same to me. When you select Opt 1, a table shows up below where you populate your project info; this is the portion I was referring to. It's been a great sanity check for our projects trying to work through the balance within this credit.
If the form you're seeing does do that, you might want to also download the sample form to test. If the one on your project website is not working right, let GBCI know via the Feedback tool that is present when you're within LEED Online. You can use the sample form and upload it to document your credit if needed.
