I am looking for some confirmation here or direction on where to look on the actual submission process.
For our project we hired a LEED acredited professional who is providing guidance. The last projet they did was in 2009. He provided us with all of the documentation they submitted and a lot of the documentation has forms that were provided by LEED. When I am on the USGG website, there are forms but I cannot download on any program or computer in our office or my home. Looking at LEEDuser it has what looks very similar to the forms from the previous project? Am I correct in thinking that most of the submission "forms" are now automated?

There are a number of resources listed there. Among them is a link to the Credit Library. If you want to see example credit forms, they will be available within the Library, where you can download them individually as "active" forms to see how they function. Once you have your LEED project website registered, the forms will be embedded for your project, and it is those that you will fill out online and provide to the reviewers for certification (along with all required supporting documentation listed on the forms). Please do note that the process for certifying a project under v4 or 4.1 is the same, but the content and requirements are significantly different. The 2009 example you have will give you an idea of what sort of details the process will require, but the majority of prerequisites/credits will be different. Hope that helps.
