Hi Tristan,
I looked at your sample documentation above and have a good idea on how to proceed with documentation. My problem is that I am not using the same program. I am using ASHRAE THERMAL COMFORT TOOL V2. I will proceed the way you did on the documentation. However, for the ASHRAE THERMAL COMFORT TOOL V2, there is a tab called the MRT (mean root temperature). The inputs here are room dimensions, 1 occupant, temperatures ect. My question is, how would I proceed to document the MRT? Do I document it on a room by room basis, or do I document it as one building. If I document it on a room by room basis, then I would have multiple cutouts of the MRT calculator to upload, which I would not want to do.

The thermal comfort parameters are documented for each space type. I work on LEED Canada projects, but it looks like the documentation requirements are similar for LEED Online.
I use a spreadsheet to track my input values (room dimensions, surface temperatures, air speeds, etc) as well as the MRT and PMV-PPD results from the ASHRAE thermal comfort tool. I include all of the parameters noted in ASHRAE 55-2004 Addendum "e". I select a representative room to analyze for each space type, and also any additional rooms with unique conditions (such as high glazing % or radiant floor slab). I then copy the summary info into the LEED submission table, and include my spreadsheet as supporting documentation. Hope this helps.
