We are working on a project in which 100% of the concrete will be regionally sourced, manufactured and harvested.
In order to document the latter, could we simply request from the contractor the the total concrete cost VS total product cost, the harvest & manufacture distance as well as an official letter from the supplier/manufacturer confirming the latter? Or is it a must to monitor and record on a spreadsheet every truck delivering materials?

Maya, it is not necesary to track every delivery. That would be intensive even by LEED standards!Simply using an invoice or another documentation of cost from the contractor along with the manufacturer attestation should be sufficient.
