We have a project that uses on site boiler for space heating and district steam for humidification. I wonder if we should still follow DES v2 and treat this building as purchase heat to build the baseline or we need to create boiler in the baseline for space heating too.

Since it is only for humidification, which must be modeled identically, I would treat it as purchased steam according to Appendix G and not bother with the DES.

Thanks for your comment. Sorry I forgot to mention we also have purchased chilled water. In this case, what do you think will be the better approach?

Hard to say without knowing all the details. So you have a choice to evaluate - Appendix G with Addendum ai and DESv2 Option 1 or 2.

For the proposed, we will have district steam for humidification, on-side gas boiler for heating. District chilled water for cooling. The question I have is how to set the baseline. DESv2 and Addendum ai only says about purchased cooling and heating. I know we will have purchased cooling, but can humidification be considered as purchased heat? If so, do we need to model baseline served by purchased steam only or also on-side boiler and district steam? Thanks for your help

Humidification is not heating. I would model the humidification identically as purchased steam. It is considered a process load. The baseline could vary depending upon the methodology you choose related to the district cooling and other factors related to Table G3.1.1A and DESv2.
