Can a project refer to one LEED AP for the Design Review Submission and another LEED AP for the Construction Review Submission? How would we document this? Submit two(2) certificates with clarifying narrative?

James, here's my 2 cents.
You could safely select either LEED AP, but since IDc2 is designated a 'Construction Phase' credit which doesn't need completion until the Construction Phase submittal, I'd probably choose your construction review LEED AP mentioned on the IDc2 form.
Re; your "submission", the 'Project Team Administrator' (or 'Project Team Manager' if you have both) will submit to, receive the comments from, and respond to GBCI. Your PTA may or may not also be your designated LEED AP.

As far as the review goes.... The credit will be reviewed during the construction review/ as a construction phase credit. I'd upload the certificate for the LEED AP for the Construction review and not get too worried about including information about the LEED AP for the Design Review. Just make sure the certificate is uploaded.
