It would be very helpful if you could provide detailed guidance on how to complete the LEED documention for projects choosing the CA Title 24-2005, Part 6 compliance path. When using the modeling approach, what specific documentation is requred for submission? Thanks for any help you can provide on this.

The v3 reference guide does not provide much detail on the documentation requirements. Therefore, v2.0 will have to be the guidence. When submitting for T24 compliance, forms PERF-1C, Env-1C, LTG-1C, MECH-1C, MECH-2C, MECH-3C, MECH-5C, and OLTG-1C are required. Therefore, this should be the T24 forms needed to document the first 2 requirements of this credit.
Forms LTG-1C and LTG-3C could be used to demonstrate the 10% LPD reduction. Need to be careful here. T24 compliance allows LPD credit for controls. I believe the LEED requirements are a 10% LPD reduction without the controls credits allowed by T24.
The Energy Star requirement can be demonstrated by submitting a table similar to Table 2 page 179 of the ref guide.

Do we still need to submit form Env-1C if our scope of work only includes changing the HVAC equipment and interior lighting? We did not touch the building envelope so did not have that part of T-24 document filled out.

If you're asking if you need to supply ENV-1C for LEED Documentation purposes for this credit if you didn't touch the Envelope, the answer is no. If you're asking about submitting it to the building department or equivalent, you'd have to ask them.
