The LOv3 Form Fix Log (https://www.usgbc.org/ShowFile.aspx?DocumentID=9144) shows that v4.0 versions of the IEQp1 and IEQc2 forms were released on 5/10/11.

I am not certain this addressed the issue because I have not tested it but I hope so.
IEQp1 says for Major Changes and Fixes on that date: “Modified calculations methods. Improved form layout, text, and navigation.” For NC, CS, SCH, and NR (retail).
IEQc2 says for Major Changes and Fixes on that date: “Modified In Scope section with linkages to IEQp1; Improved form layout, text, and navigation.” For NC, CS, SCH, and NR (retail).

IEQp1 was updated this week to include the built-in MZ calculator. Project teams with the v04 version of the form received this update automatically (no action is required). The sample form will be updated to reflect these changes next week.
Documentation options for multi-zone systems therefore include: the ASHRAE calculator (xls), the LEED version of the ASHRAE calculator (xls), and the built-in calculator.
