I have been told by our very reputable Design firm that is is not unusual to have two design appeals on the EA credit 1 and prerequisites. I am losing faith and would like to know what everyone else's experience has been on this? Is a third appeal unheard of?

A small percentage of projects do have to go through a second appeal. Third appeals are pretty rare - I have never seen one.
Most of the problems are not following the Appendix G methodology correctly. The other major reason I see for appeals are very cavalier and incomplete responses to the preliminary review comments. Generally one word or even one sentence responses will be inadequate. In response thoroughly explain what you did originally, what you did to fix it (if you had to do so) and provide evidence of the changes made.
Appeals are entirely avoidable by making sure to follow the rules and producing results which make sense. So far (knock wood) we have submitted a couple of dozen models for LEED project and have not had to any appeal yet.
