Solid Waste Management Reference Form
While not required for LEED documentation, this is a customizable form that projects can use help anticipate how to deal with particular types of waste specific to a facility alteration or addition project. Use this form to track cleaning equipment maintenance, and to demonstrate that equipment was maintained throughout the performance period.

Use this spreadsheet to inventory your cleaning equipment and evaluate it against relevant sustainability criteria. A sample inventory is included, along with the basic credit criteria.

Air filters must be MERV 13 or better. Use product cut sheets like this example to find credit-compliant products, and to evaluate their energy efficiency.
Occupant Comfort Survey
Use a brief survey like this one to collect occupant feedback on satisfaction with building conditions, including temperature, acoustics, indoor air quality, lighting levels and building cleanliness. Customize the survey for your building to gain useful information.

These sample surveys may be tailored to meet your project building’s needs. Use them to gather occupant commuting data. One of these templates is intended for electronic distribution; one is intended for gathering information in the building's lobby in a "lobby blitz."
