I've not submitted an O+M project before and was looking for a sample preventative maintenance plan to get an idea of the level of detail the reviewer is looking for. The client uses an automated order system that keeps them coordinated, but there's not a *narrative* type document with verbiage like what I think is in the pre-req's intent.
