How to Submit a LEED Interpretation

I'm teaching a LEED GA study group and was rambling on the differences between CIR and precident-setting LEED Interpretation (that gets posted in the addenda library and all). Then I went onto LEED Online and was showing how those are submitted... but can't actually figure out how a LEED Interpretation would be submitted. Has anyone done this recently who can explain it to me (so I can show my group)?  

These instructions seem pretty specific to a CIR: https://www.usgbc.org/resources/how-submit-credit-interpretation-request-leed-online

But it does have an obscure line (under #4) that reads: 
Please note if you are submitting a LEED Interpretation, choose the applicable rating system, and the applicable MPR or Pre-Requisite. If the credit is not listed, choose MPR1. Include “LEED INTERPRETATION” in the start of your inquiry. Please be sure to select “pay by check” as your payment method.

But then there are no other instructions or mentions of how much to write the check for or who it should go to... maybe that all comes later in an email?

This is obviously not something I've ever done, but would love to clarify for my group if possible...  Anyone submitted for a LEED Interpretation recently?
