No. Only provide those pages that support the achievement of each credit located behind the respective template page with the pertinent information HIGHLIGHTED clearly. If the documentation applies to more than one credit, individual copies should be filed with each credit if but a few pages. When the attachment is more than a few pages and applies to more than one credit, include just one copy and reference it for each credit. For each credit, it would be most helpful to use color-coded highlighting to distinguish the information for several credits on that one copy.
