Has anyone achieve the credit Safety First: Re-Enter Your Workspace

For the daily journal with building occupants, below questions were examples, But can it be more specific to COVID and brief?

  1. Occupants:
    1. Did you feel safe in this place today?
    2. Are you aware of the plan for safety of this place?
    3. Were you able to social distance?
    4. Did others follow the rules?
    5. Are you aware of anyone who became ill in this place?
    6. Are you comfortable in this place?
    7. Are you in control of your life?
    8. Were you able to practice wellness practices at this place?

My suggestions are

Name, Temperture, Have you been in contact with COVID-19 patient past two weeks?, do you have fatigue? .... etc.
