Hi Marie - for (b) you should post on the MRpc87 LEEDuser forum (observations about the process or requirements, ideas for improvement, questions, etc) and take a screenshot of the post to include in your application. For (c) you should get a survey completion email that you can include in your application.

Proof that the facility or facilities that received materials from the project was "certified" over the timeframe that waste was generated on the project and sent to the processing facility.
Calculations that demonstrate that the required percentage of commingled construction waste is sent to a facility that has a current* third-party verified facility recycling rate.
- *For the purposes of the credit “current” shall be defined as a facility in good standing with the certifying body at the time of project construction.
Recycling rates applied to a project's waste generation must be the weighted average for the facility over the time in which the project sent materials to the facility. Time increments shall not be reported less than quarterly and no more than annually, and must use consistent time increments throughout calculations.

It's not a survey that they send you; there's a link to the associated survey on the webpage that has the credit requirements. Once completed and submitted, you'll receive a confirmation email, which you should then PDF and provide as an upload.
Word of advice, make sure you put your own email at the start, as whoever is listed there will be the one to receive the confirmation email. I learned this the hard way.

I pursued this Pilot credit on a project in California who utilized RCI facilities for co mingled recycling and this credit seems easy to attain for project with access to certified co mingling facilites. These are not always available depending on the location of your project, so that can be a challenge.
