The most recent LEED Online form I'm looking at asks for the plan to be uploaded but then only asks you to "Summarize how each operational element was implemented in an environmentally sensitive manner during the performance period." Does this mean uploading the tracking for these exterior maintenance categories is not required?

In general it should be okay to follow what's required on the form version you have in LEED Online. That said, for this particular credit I'd stress that it's important to provide a narrative that's specific and includes quantitative metrics (wherever possible) if you forego uploading an activity log. GBCI prefers that teams don't provide performance statements that are too general when the metrics included in the plan are quantitative.
