In the purchasing policy template document provided in the documentation toolkit, the proposed percentage is "75% meet sustainability criteria", where did this percentage come from and what is the minimum accepatble percentage?

The proposed percentage comes from the requirements of the associated credit. Regarding the minimum acceptable percentage, earlier this year I have the same inquiry, and wrote to the GBCI about it, this was the answer:
"Goals do not necessarily have to meet the minimum thresholds in MRc Purchasing or EQc Green Cleaning and do not need to match those in the sample policies, but the sustainability criteria of the credit and all applicable material types must be addressed. Be sure that the policy establishes quantitative goals and performance metrics for each aspect.
Note, at the top of the sample MR policy it says:
"This policy example meets the requirements of LEED v4 O+M MR Prerequisite Facility Maintenance and Renovation Policy. The contents of this policy, including but not limited to the policy scope and goals, roles and responsibilities, standard operating procedures, implementation strategies, performance measurement and schedule for reassessment, and quality assurance, will vary by project based on the building’s circumstances. Be sure to customize this policy example, tailoring it to your project’s specifics. It will be reviewed as a part of your project’s documentation submission."
