This sentence is extracted from a sample CWM Plan, so should I submit expected or real quantities of materials diverted from landfill? i.e. I should submit a PLAN or a REPORT?
If submit a PLAN, how to ensure that expected quantities tally with real quantities?

You should actually submit both. You will need to upload your construction waste management plan in the "file uploads" section of LEED Online and then you will also need to fill out the table on the credit form that tracks all of your construction waste and asks whether it was sent to a landfill or diverted from a landfill through recycling or salvage.

Xing, it sounds like you are asking about what to submit when. You will want to prepare the Construction Waste Management Plan prior to starting construction as a means to identify what types of waste will be generated, how it will be handled, and where it will go. This will help you in budgeting and arranging haulers in advance as well as setting up your job site for collection, sorting, and pickup procedures. Be sure this CWM Plan requirement is included in the Construction Documents.
During the construction process, you will then have the actual data for weight (or volume) of waste which you will track on a regular basis. Refer to the LEED Online LEED MRc2 credit form itself if you have a registered project to see exactly what is expected for the “actual” reporting portion. It is a similar “chart” to what you have referred to, but a weight column is added to report quantities for each waste type. Starting with the end in mind, you might create your in-house tracking forms to resemble the LEED Online format.
And yes, you will ultimately be submitting both your CWM Plan (as an uploaded document) and filling out the MRc2 LEED credit form, with similar base information, but including the weight (or volume) of waste types here. The form automatically calculates how much was diverted or land-filled based upon your inputs.
