Our CI project has a large amount of re-used furniture, which we will need to provide cut sheets for in order to get to the min 20% threshold. What would qualify as a cut sheet, if the piece of furniture is old and re-used? Can we provide a statement on letterhead that certifies that the furniture came was re-used? (Previous location is within 500 miles)

Katie, I'm surprised that there's been no reply... this is what we're doing on a project that we're about to submit to fulfill the cutsheet requirement I'll follow-up after our review & let you know if that is NOT allowed.... assuming you're not in submittal phase as well.

We've followed that same strategy in the past with success. All you need to do is upload a statement attesting to where the furniture was salvaged (considered to be the harvest/extraction location). If you did any sort of refinishing/refurbishing, then that location is considered to be your manufacturing location (and so would also need to meet the 500-mile requirement). Typically we will also specifically note in our letter if the reused pieces had any refurbishing/refinishing done as that directly impacts whether those pieces must be included in IEQc4.5 as well. That way, the one letter can serve as documentation for both credits for those pieces of furniture.
It is important that the letter be provided by whoever is supplying the furniture (either the Owner if they're reusing their own items or the supplier if its a salvage company) and that the letter is signed+dated and on letterhead.
Hope that helps!

Thanks for the responses. Very helpful Kristina! I've been away on leave, so sorry I didn't say something sooner. We will be submitting soon - so I will followup if necessary.
