Previously, We have been a part of projects in which LEED credit has been given for the re-use of concrete, both structural and flat work, used to backfill an old gravel pit. The gravel pit was eventually developed as a residential development.
Does LEED NC v.4 allow for this credit to the site of demolition?

Chad - Yes - it should be allowed to be counted as diversion. This credit still covers demolition and construction waste. There are several changes from LEED 2009 including excluding ADC (alternative daily cover) for landfills but since your situation is for a gravel pit and not a landfill, I believe it will count.

Chad - While reviewing the LEED v4 Reference Guide (under Further Explanation in MRp2) yesterday, I found this statement, which is a change from what we've seen in LEED 2009: "Use of construction waste to infill mining pits is permissible only if the waste is “clean” and the work is overseen by the state or local government or a government sponsored organization." The Definitions section defines clean waste as "nonhazardous materials left over from construction and demolition. Clean waste excludes lead and asbestos." Please ensure your project takes this into consideration.

Any updates or clarifications on using a commingled recycling facility?

Ralph - My apologies in not responding sooner. I somehow got unsubscribed from this forum.
Several LEEDusers have gotten Project CIRs that indicate that the USGBC is not accepting commingled waste as multiple streams unless the facility can provide project-specific diversion rates per this post string - http://www.leeduser.com/comment/redirect/69734- further up on this page. Earlier this year, several of us reached out the MR TAG Chair who said he would ask staff to include this on this year's MR TAG agenda. One of the group is working through LEED Coach also but I don't have any more specifics. I've been waiting to edit this page's content until we have a definitive answer that is a LEED Interpretation or Correction to the Rating System.
Sorry I can't be more help.

I wanted readers of this thread to know that we have updated LEEDuser's Viewpoint for MRp2 and MRc5 (MRc6 for ID+C) to reflect the issues teams have experienced with LEED v4 and commingled waste.
New/revised MRc5 language states: "If the commingled recycling facility can track and produce documentation of specific materials recycled for your project, you can count commingled waste as multiple waste streams. Otherwise, commingled waste that is the average diversion rate for a regulated facility is counted as a single waste stream regardless of how many different materials are included. The average recycling rate for the facility must exclude ADC."
In addition, we added a LEED v4 resource for converting volume-to-weight for construction and demolition debris - https://www.usgbc.org/node/2695214?return=/credits/mid-rise/v4.
FYI: The URL listed in my May 2017 post is https://leeduser.buildinggreen.com/forum/filling-out-cd-waste-calculator#comment-49374 in the new (September 2017) LEEDuser format. I have added the comment above regarding commingled waste to that forum as well.
