I am unable to "CHECK FORM" at the bottom of my template IEQc5 in LEED online. The text "Incomplete. Please complete the highlighted fields and/or address any omissions in the Special Circumstances section." appears next to "CHECK FORM", but there are not any highlighted fields or omissions in the form. Does anyone know how to fix this, or, how to explain this to GBCI when I submit? It seems like an error in the form itself.

Not aware of errors in this form specifically, but you can always add a note in the Special Circumstances box stating something like "form says it is incomplete but I have verified all fields are completed" if you come across an error like that. As long as the reviewer has all the information they need, they won't pend/deny for form issues like that.

Thank you! I was able to talk with GBCI on the phone this morning. They were also not able to detect why the "CHECK FORM" button is not functioning. GBCI stated that I should explain the issue in the Special Circumstances box of the template, and submit the form, as you suggest above.
