No Smoking Signs

Where do the no smoking signs need to be located? All exterior doors? Only ingress doors?

My understanding is that the Owner must place her/his initials in the Signatory box for credits such as this one (No Smoking). Please correct me if I am wrong, but the Mechanical Engineer is not...

Owner would prefer if the sign did not have the "noisy" icon of a "ghostbusters" red circle with line through it and a cigarette. Would prefer if it was just text stating "This is a clean air...
