Here's what I can tell you: Have you looked at the sample form for this prerequisite- available here http://www.usgbc.org/sampleforms or in the Credit Library? As outlined in the Reference Guide, the v01 version of the form, and your inquiry, the only required upload is the CWM Plan. You also have to provide total construction waste as well as total waste diverted...
